IFHO — What Happens

What Happens

From First Conversation to Your Plan

Four steps, roughly two months end to end. Most of that is laboratory time — whole genome sequencing and multi-omic analysis cannot be rushed without compromising what they tell us.

  1. 01

    Consultation

You meet with Anil Bajnath, MD to review your history, your goals, and what you want the data to answer. Together you determine which program fits.

    In person or telemedicine

  2. 02

    Specimen Collection

Collection is matched to your panel. Some programs need a single blood draw; others require several visits or additional sample types such as saliva. Elite and Concierge members can have collection arranged at their home, their place of work, or another convenient location within thirty miles of our office.

One or more visits · Nearest lab, or arranged around you

  3. 03

    Analysis and Interpretation

The laboratory processes your specimen while Dr. Bajnath builds your threat map and hallmarks profile, reading every result against your history rather than a population reference range.

4–6 weeks from the date the lab receives your specimen

  4. 04

    Results Review

You sit down with Dr. Bajnath and walk through the findings in full — what is stable, what is compensating, what needs attention now — and leave with your detailed report, your precision plan, and your digital twin.

Detailed report · Precision plan · Digital twin

Elite and Concierge continue from here with ongoing testing, monitoring, and recalibration. The Biological Audit® on its own concludes with your report and plan.

IFHO — From Insight to Intervention

From Insight to Intervention

From Biological Audit to Precision Action Plan

The Biological Audit® is not a report. It is a roadmap. Once we have mapped your biomarkers, multi-omic signatures, and hallmarks of aging, we translate those findings into a precise, individualized strategy designed to improve your trajectory of health.

This is where data becomes medicine.

  1. 01
Define Your Biological Priorities We organize your data into clear priorities: which molecular processes, organ systems, and aging mechanisms require immediate focus, and which are currently stable or outperforming your chronological age.
  2. 02
Design Your Precision Intervention Strategy We architect a personalized plan across lifestyle, pharmacology, peptides, regenerative biologics, supplements, and environmental shifts — ranked by clinical impact and feasibility.
  3. 03
Target the Drivers of Your Aging Pattern Every recommendation is mapped back to the hallmarks and mechanisms most active in your data — mitochondrial efficiency, senescence, inflammation, nutrient sensing, genomic stability, and more.
  4. Steps 1 through 3 complete the Biological Audit®. Steps 4 and 5 continue with Elite or Concierge.

  5. 04
Implement With Physician-Led Guardrails You receive structured protocols, dosing guidance, and sequencing with physician oversight, so each intervention is implemented safely and in alignment with your biomarkers.
  6. 05
Track, Reassess, and Recalibrate Your biology is dynamic. We repeat selected testing, monitor key biomarkers, and refine your plan as systems adapt — keeping your trajectory moving toward greater resilience and improved healthspan.
IFHO — What It Is

What It Is

Your Body, Read as One System

Most evaluations look at the body one symptom or one lab panel at a time. The Biological Audit® looks at how your systems behave together — how metabolism informs hormones, how immune activity shapes your aging trajectory, how gut and mitochondria influence cognition, how genes and environment interact over years.

Anil Bajnath, MD built it across a decade of treating complex metabolic, inflammatory, and age-related conditions, drawing on the systems-biology frameworks he teaches at George Washington University. He personally oversees every audit, so each data point is read against your physiology, your history, and your goals.

IFHO — What We Measure

What We Measure

From Molecules to Organs

Two views of the same biology. First how your organ systems are performing, then the molecular layers underneath that explain why.

Organ systems

  • Brain and cognition Focus, memory, mood, neuroinflammatory signals.
  • Cardiometabolic health Blood vessels, lipids, glucose control, inflammation.
  • Immune and inflammatory balance Cytokines, chronic activation, immune resilience.
  • Hormonal signaling Thyroid, adrenal, and sex hormones affecting energy, mood, and composition.
  • Gut and microbiome Digestion, barrier integrity, nutrient absorption, microbial ecology.

Molecular data layers

Foundational layer

Genomics

Your inherited blueprint — SNP and polygenic risk patterns, pharmacogenomic signatures, detox and inflammatory genes.

Biological aging

Epigenomics

DNA methylation patterns and biological age estimates showing how lifestyle and exposure have shaped gene expression.

Real-time activity

Transcriptomics

mRNA expression trends, cellular stress responses, and immune activation patterns reflecting how your cells are responding now.

Signaling layer

Proteomics

Cytokines, growth and repair factors, and vascular proteins that reveal inflammation and nutrient-sensing activity.

Metabolic function

Metabolomics

Organic acid profiling, mitochondrial efficiency markers, and fatty acid balance — a biochemical fingerprint of your metabolism.

Gut ecosystem

Microbiomics

Microbial diversity, dysbiosis and barrier integrity, and the metabolites your gut ecosystem produces.

IFHO — Hallmarks of Aging
HALLMARKS OF AGING

Biological Age

Twelve Hallmarks, Measured Individually

Rather than reducing you to a single biological age number, we evaluate each of the twelve recognized hallmarks of aging against your multi-omic and clinical data — identifying which processes are stable, which are compensating, and which have begun to accelerate.

Primary — the damage itself
Genomic instability · Telomere attrition · Epigenetic alterations · Loss of proteostasis · Disabled macroautophagy
Antagonistic — the body's response
Deregulated nutrient sensing · Mitochondrial dysfunction · Cellular senescence
Integrative — what you feel
Stem cell exhaustion · Altered intercellular communication · Chronic inflammation · Microbiome disturbance
Read how we measure each hallmark
IFHO — What We Find

What We Find

Your Findings, Ranked by What Matters

The outcome is not more data. It is a map of the biological threats that matter most to your health, performance, and aging — six domains, each reported on the same three-point scale.

  • Metabolic and cardiometabolic Early changes in glucose control, insulin dynamics, lipids, visceral fat, and vascular inflammation that raise risk for diabetes, cardiovascular disease, and metabolic syndrome.
  • Inflammatory and immune Patterns of chronic low-grade inflammation, immune over-activation, or immune exhaustion that influence autoimmunity, infection risk, and the pace of aging.
  • Cardiovascular and vascular Signals related to endothelial health, plaque formation, blood pressure patterns, and microvascular function that shape long-term heart and brain health.
  • Neurologic and cognitive Markers connected to brain energy, neuroinflammation, sleep quality, mood, and cognitive performance that can signal decline long before symptoms appear.
  • Hormonal and stress system Thyroid, adrenal, and sex hormone patterns, along with stress system load, that affect energy, weight, recovery, mood, and long-term resilience.
  • Aging trajectory and resilience Biomarkers of aging, recovery capacity, and system flexibility that reveal whether you are aging faster, slower, or in line with your chronological age.

This map becomes the foundation for your strategy — what to address first, what to protect, and how to move from compensation to optimization.

Investment

Programs & Investment

The Biological Audit® is the diagnostic physical exam of the future. Where the annual checkup looks at a handful of surface markers, the Audit reads your biology across multiple omic layers — genomic, epigenetic, proteomic, metabolomic, and microbial — interpreted together by a physician and translated into a precision plan. Every program begins with a consultation, and capacity is deliberately limited.

Beyond the annual physical

A standard exam measures roughly a dozen values. The Audit evaluates thousands of data points across systems that interact.

Multiomic by design

Genome, epigenome, proteome, metabolome, and microbiome are read as one integrated picture rather than isolated reports.

Measured, not inferred

Every recommendation traces back to a measured value, and every value is re-measurable so change can be tracked over time.

What does multiomic actually mean?

An "ome" is a complete set of one type of biological molecule. Multiomics is the integrated analysis of several of those layers at once — the genome (your DNA sequence), the epigenome (chemical marks that regulate which genes are switched on), the proteome (the proteins your cells are actually producing), the metabolome (the small molecules produced by metabolism), and the microbiome (the bacterial populations in your gut). Each layer answers a different question: your genome describes predisposition, while your metabolome and proteome describe what is happening right now. Analyzed in isolation, they are separate reports. Analyzed together, patterns emerge across layers that no single test can show.

Are labs included in the fee?

Yes. Advanced omic testing is included in your audit. The fee also covers physician time, interpretation, and program design. Optional add-on testing and imaging are billed separately.

Is the membership required?

No. The Biological Audit and Biological Audit Elite are complete one-time evaluations. The Ongoing Optimization Membership is an optional continuation for those who want quarterly re-measurement and physician-guided adjustment, and it is priced separately.

Why is capacity limited?

Each program requires direct physician time for interpretation, strategy, and ongoing adjustment. Concierge is capped at fifty patients per year so that level of attention remains possible. Audit and Elite open as capacity allows.
