How the Membership Works

A year of intentional medical stewardship

This membership is designed for individuals who treat health like an asset class. You don’t need more medicine — you need better decisions, made earlier, with a physician who understands your baseline and tracks your trajectory over time.

12-month partnership with defined cadence
Longitudinal oversight not episodic visits
Actionable interpretation not a lab dump

The rhythm of the year

A predictable clinical cadence with quarterly checkpoints — built to measure progress, detect drift early, and keep strategy aligned with real-world demands.

Baseline & Strategic Design

Establish your reference physiology and define what “optimal” means for your goals.

  • Comprehensive biomarker baseline (metabolic, cardiometabolic, inflammatory, hormonal, recovery)
  • Performance reserve assessment (VO₂ max & physiologic capacity)
  • Strategy design: priorities, sequencing, and what not to do

Early Adaptation & Signal Detection

Identify response patterns, tolerance, and early drift before it becomes dysfunction.

  • Quarterly labs to verify trajectory (not assumptions)
  • VO₂ max trend + recovery/stress review
  • Adjustments: escalation, de-escalation, or maintenance

Optimization & Stability

Preserve gains and performance under travel, stress, training demands, and real life.

  • Quarterly clinical checkpoint + biomarker review
  • VO₂ max reassessment to track reserve
  • Therapy and program alignment based on readiness

Reassessment & Year-Ahead Planning

Integrate the year’s data and set the next cycle with precision.

  • Trend analysis across biomarkers + performance
  • Risk architecture review and prevention planning
  • Strategic roadmap for the next 12 months

Concierge Longevity Membership

Who this membership is designed for

Concierge care at the Institute for Human Optimization is built for individuals who want more than access — they want judgment, expertise, and continuity.

This membership is ideal for individuals who:

  • Want a primary physician relationship guided by advanced longevity and functional medicine training
  • Value clinical decisions informed by systems biology, aging science, and performance medicine
  • Prefer proactive oversight rather than reactive, episodic care
  • Expect thoughtful interpretation of complex data — not automated protocols
  • Want care that integrates prevention, optimization, and traditional medicine

This approach resonates most with people who:

  • Understand that longevity medicine requires specialized training and judgment
  • Value a physician fluent in both conventional medicine and emerging longevity science
  • Prefer evidence-based optimization over trend-driven biohacking
  • Want continuity with a physician who understands their baseline and trajectory
  • Believe expertise matters as much as access

Why this membership is different

Unlike many concierge practices that focus primarily on access, this program is led by a physician with advanced training in functional medicine, longevity medicine, and performance optimization. This allows care to extend beyond routine primary care into proactive, data-driven strategy — often the very reason individuals seek concierge medicine in the first place.

Concierge Medicine — Frequently Asked Questions

Concierge care at the Institute for Human Optimization is designed differently. These answers clarify how the program works, what’s included, and why this model exists.

What is Concierge Medicine at the Institute for Human Optimization?

Concierge Medicine at the Institute for Human Optimization is a longitudinal, physician-led partnership focused on healthspan, performance, and risk management — not episodic or reactive care.

Members receive structured clinical oversight, strategic diagnostics, and direct access to Anil Bajnath, MD, within a deliberately limited, non-volume practice.

Who is this program designed for?

This program is designed for individuals who value:

• Proactive, data-driven health management
• Advanced training in longevity, functional, and performance medicine
• Continuity with a physician who understands long-term trajectory
• Strategic decision-making rather than protocol-driven care

Many members are founders, executives, professionals, or individuals optimizing for long-term resilience and performance.

What is included in the Concierge Membership?

Concierge Membership is comprehensive and intentionally structured. It includes:

• Annual or baseline Biological Audit (for new patients)
• Quarterly laboratory assessments
• Quarterly VO₂ max and performance evaluations
• Mobile phlebotomy within 30 miles of 21076
• Continuous monitoring integration (CGM, wearables, biometrics)
• Direct physician access and concierge patient coordination
• Premier Longevity Gyms access with physician-guided recommendations
• Preferred pricing on weight loss, hormone, peptide, and regenerative programs

Medications and procedures are prescribed separately when indicated and are not bundled into membership fees.

How does the Biological Audit relate to Concierge Membership?

For new patients, Concierge Membership includes a comprehensive Biological Audit as the foundation of care.

Patients who complete a Biological Audit and transition into Concierge Membership within the same year receive a partial credit applied toward the annual membership.

This ensures continuity while recognizing the investment already made in establishing a detailed biological baseline.

How is advanced imaging handled?

The Institute for Human Optimization partners with premier regional imaging centers, including Charter Radiology, for advanced diagnostic imaging when appropriate.

Imaging is recommended strategically based on clinical presentation, goals, and risk profile. Elective imaging studies are billed separately by the imaging provider.

How many concierge patients does Dr. Bajnath accept?

Dr. Bajnath maintains a very limited concierge panel to preserve depth, responsiveness, and white-glove access for members.

New patient enrollment is capped each year, with priority always given to existing members. Availability is limited by design.

Do you accept insurance, HSA, or FSA funds?

The Institute for Human Optimization operates as an out-of-network, physician-led practice.

We do not bill insurance directly. Many services and diagnostics may be eligible for HSA or FSA reimbursement depending on plan rules. Super bills can be provided when appropriate.
