
VO₂ Max: A Window Into Cardiovascular Fitness and Longevity
Blood pressure, cholesterol, resting heart rate, and laboratory testing provide important information about cardiovascular health. But they don't tell us everything.
One of the most valuable questions we can ask is:
How well does the cardiovascular system perform when demand increases?
That is where cardiorespiratory fitness, and VO₂ max specifically, becomes particularly valuable.
What Is VO₂ Max?
VO₂ max represents the maximum amount of oxygen the body can take in, transport, and utilize during progressively intense exercise. It reflects the integrated performance of several systems.

The lungs must bring oxygen into the body. The cardiovascular system must transport oxygenated blood. The heart must generate sufficient cardiac output. Blood vessels must distribute that blood effectively. And skeletal muscle must extract and utilize oxygen to produce energy.
VO₂ max therefore isn't simply a measure of athletic ability. It is a measure of integrated physiological capacity.
Why Cardiorespiratory Fitness Matters
Cardiorespiratory fitness is strongly associated with health outcomes. Large observational studies have consistently demonstrated an inverse relationship between cardiorespiratory fitness and all-cause mortality: individuals with higher fitness levels generally experience lower mortality risk than individuals with poor cardiorespiratory fitness.
Importantly, the relationship isn't relevant only to elite athletes. Improving fitness from a low baseline can be meaningful.
This makes cardiorespiratory fitness particularly interesting from a preventive-health perspective because, unlike many risk factors we cannot modify, fitness is trainable.
VO₂ Max Changes With Age
Aerobic capacity generally declines with aging. The rate varies considerably between individuals and is influenced by genetics, physical activity, cardiovascular health, muscle mass, training history, and other factors. That makes maintaining cardiorespiratory fitness an important component of healthy aging.
The objective isn't necessarily to achieve an elite VO₂ max. For most people, the more meaningful goal is maintaining sufficient physiological reserve to continue doing the things they want to do as they get older.
Walking long distances. Climbing stairs. Traveling. Exercising. Playing with children or grandchildren. Remaining independent.
Longevity without physical capacity is an incomplete goal.
Why Measuring VO₂ Max Is Different From Estimating It
Wearable devices and fitness algorithms can provide useful estimates of VO₂ max, but laboratory assessment provides a different level of information.
During a VO₂ max assessment, respiratory gases can be measured while exercise intensity progressively increases. The test evaluates oxygen consumption and carbon dioxide production, providing objective information about the body's physiological response to exercise.
Depending on the testing protocol and equipment used, this information can also help characterize aerobic capacity and inform individualized exercise intensity.
Instead of simply asking, "Am I exercising enough?" we can ask: "What is my current physiological capacity, and how should I train to improve it?"
Exercise Should Be Prescribed With Greater Precision
Generic exercise recommendations are valuable at a population level, but individuals respond differently to training. Age-based heart-rate formulas are estimates. Two people of the same age can have very different fitness levels, physiological responses, and training needs.
Objective testing allows exercise recommendations to become more individualized. For performance-oriented individuals, this can improve training efficiency. For individuals focused on longevity, it creates a measurable baseline that can be reassessed over time.
Don't guess your cardiovascular fitness. Measure it.
